Filling in various Surveys issued by buyer

MCD stands for Material Composition Declaration, consisting of the Certification of Compliance, Composition Datasheet and Test Report. Buyer issued the MCD Survey to suppliers for Green Product data collection.

Please note that the MCD Survey are issued by your buyer. You may not add it on your end.

My To-Do List

You could search for the MCD Survey issued by buyer from the following path:

1. ezGPM.NET platform > [To-Do List] on the system main page [To-Do / Bulletin].
2. ezGPM.NET platform > Click [MCD Survey] under Edit and Reply from the sidebar menu.
3. Additional reminder emails [ezGPM.NET To-Do List] will continue to be sent from the system before all cases on the [To-Do List] are completed.

Fill in MCD Survey

Please follow the steps below to fill in the MCD Survey:

1. Click [MCD Survey] under Edit and Reply from the sidebar menu.

2. From the MCD Survey task list where all Part No. are displayed, click the Part No. to enter the editing page of the MCD Survey you are working on.

3. The first section is Basic Info.. You may check the Part No. and other information that your buyer set it for you. Please fill in the required fields with red asterisk* mark. If there are no required fields, please go to the next section.

You may use the [Copy] function to copy the data you have filled in the MCD Survey. Please click , then search for the MCD Survey you want to copy.

Note:

1. All Raw Materials and documents referenced will be copied.
2. Original data will be replaced by the copied data. We recommend that you copy the [Approved] MCD Survey as they are already verified and approved by buyer.

4. The second section is Certification of Compliance. Select the existing file which is already created in the Document Center from the drop-down menu, or insert the file name in the column to search. You may also click to create one. The one created will automatically be referenced.

When the [Required] section shows [Yes] with red asterisk, you will need to provide the Certification of Compliance. If is shows [No] or you do not see this section, no Certification of Compliance is required by your buyer.

5. The third section is [Raw Material]. Once you click , there are 4 sections that are required to be filled.

6. In the first section, please Fill in Basic Info. of the following information:

• Sub Part Name
• Raw Material Name
• Material Category
• Raw Material Weight (g)

1. If you can't find the suitable material category, you can send an application by clicking [Apply] on the right side of the field.
2. If there is/are Exemptions that is/are applicable for the Raw Material, please select the [Exemption].

7. In the second section, please Upload Composition Datasheet. Select the existing file which is already created in the Document Center from the drop-down menu, or insert the Material Original Maker or Material Spec. in the column to search.

Only one Composition Datasheet could be selected for each Raw Material.

If you haven't created the Composition Datasheet in the document center, you may click to create one. The one created will automatically be referenced.

8. In the third Section, please Upload ICP Report. Select the existing file which is already created in the Document Center from the drop-down menu, or insert the Report No. in the column to search.

Click to check the Substances required to be tested in your test report.

You may enter the keywords such as the Report No. to search for the report, more than one report could be referenced.

If you haven't created the ICP Report in the document center, you may click to create a one. The one created will automatically be referenced.

9. The fourth section is Declare Guaranteed Substances. You may click to view the substances that are [Guaranteed] not contained in the Part. Should you agree and guarantee, please check the box.
10. When completed, click and to go to the Compliance Analysis page. If everything is compliant (no red note shown), click to go back to the main page.

Should there be red note shown, please follow the note and click to make modifications.

11. Should there be more Raw Materials, please follow Step 6-10 to create more Raw Materials. When completed, click the button on top to submit the survey to your buyer. Once submitted, you have completed the survey.
