Introduction to Fill in REACH SVHC Survey

The REACH form provides an efficiency interface for REACH SHVC declaration, allowing suppliers to quickly complete survey task.
Instructions for operation
The REACH form survey is published by the system factory, you can query by the system's to-do/announcement screen, or directly from the REACH form to fill out the main screen query, the following steps:
- Log in to the ezGPM.NET platform, click on the left function column to "Edit and Reply > Reach Form" function, open the REACH SVHC survey form on the right screen .
- Click
queries all REACH SVHC forms, or sets criteria in the field above to query specific REACH form records. - In the list of query results, click the blue-letter link of the first field, the "Associated Central site", to enter the reply screen.
If your material does not contain REACH highly concerned substances, after you review the list of substances below, attach the certificate file (if there is a test report), click
above to complete . - If your material contains REACH highly concerned substances, please select the "YES" field of the substance, and directly fill in the containing values, parts and reasons, to be complete dislocated after the data click to complete .
Notice
- The latest release of SVHC Candidate List ( substances of very high concern) is automatically updated in accordance with REACH regulations, and you can view the current version in the "REACH version" field above the form.
- The test report can only upload one file, if you have more than 2 files, please merge into one file and upload it later.
- The REACH form determines whether the file of the left certificate needs to be uploaded according to the requirements of the Central site.
